Our medical services include: primary care, annual checkups, well-child exams, physicals (sports, kindergarten), immunizations, women's health services, lab work, diabetes and chronicdisease management, stitches and wound care, prenatal and postnatal care, and cancer screenings.
Through our partnerships, qualifying patients can access reduced-rate imaging services including MRI, CT scan, ultrasound, X-ray, and mammography.

Oral health is essential to your overall health. Creek Valley Dental Clinic provides comprehensive dental services including cleanings, fillings, extractions, emergency care, root canals, and crowns. Our team also provides fluoride varnish, oral health education, and personalized oral care plans.

A healthy mind helps you realize your full potential, maintain relationships, and handle life's challenges. Our behavioral health team provides counseling, evaluation, therapy, and psychiatric services tailored to each patient's needs.
We help with: stress, depression, anxiety, behavioral issues, relationship struggles, family crisis, domestic abuse, managing health challenges, psychological testing (including autism spectrum screening), and psychiatry services.

Physical Therapy
At Creek Valley, your recovery is a partnership. We provide one-on-one, expert-led care tailored to your unique goals, whether you're healing from injury, managing chronic pain, or working to move and feel your best.
Our PT services include: orthopedic rehabilitation, balance training, sports medicine, dry needling, neurological therapy, vestibular treatment (dizziness/vertigo), chronic pain management, TMJ treatment, and postpartum care.

Substance Use Treatment
Struggling with alcohol or opioid misuse? Creek Valley offers Medication Assisted Treatment (MAT), which combines FDA-approved medications with counseling to help you focus on recovery.
Available medications include Buprenorphine, Naltrexone, Suboxone/Subutex, and Vivitrol. We also provide free Narcan for overdose reversal.
We host a free recovery support group meeting every Tuesday at 5pm in our conference room. This patient-centered support group welcomes anyone wanting to share and receive support in recovery.

Pharmacy Services
Creek Valley has partnered with Bee's Marketplace through the 340B Pharmacy Discount Program to support medication access. Patients can access prescriptions at steep discounts, especially for insulin, inhalers, and chronic disease medications. Learn more at hrsa.gov/opa.
