Benjamin Williams, DO
Dr. Williams oversees all providers at Creek Valley Health Clinic. A Colorado City native, he completed his medical education at Mayo Clinic in Rochester, MN. With extensive experience in pediatrics, adult medicine, and geriatrics, Dr. Williams brings deep commitment to community-centered care. He lives in Centennial Park with his wife and children and is currently accepting new patients.

Joanne Yarrish, CNM, FNP
Joanne oversees much of our OB/GYN care, providing prenatal and postnatal support, women's health services, and family planning. She also works with the Centennial Park Birthing Clinic. With more than 30 years of clinical experience and over 1,000 deliveries, Joanne is dedicated to supporting women at every stage of life. She typically sees patients 2-3 days per week.

Rebekah Williams, PA-C
Rebekah graduated from the University of Utah's Physician Assistant Program in 2023 and brings 14 years of prior experience in Hyperbaric Medicine. A native of the Short Creek area, she's grateful to be back home serving the community she loves.

Marianne Holmes, FNP-C
Marianne earned her MSN from United States University in 2024 after six years of hospital nursing experience. Passionate about whole-person wellness, she empowers patients to understand their health and make informed choices. A St. George resident, she's proud to serve her community with compassionate, personalized care.

Matt Lasslo, LPC
Matt brings diverse experience in mental health, beginning as a crisis counselor and later working in outpatient settings. He has extensive experience helping clients with depression, anxiety, PTSD, and substance use challenges. Originally from Arizona, Matt is committed to integrating mental and physical health for positive outcomes.

Peggy Taylor, Psy.D.
Dr. Taylor is licensed in Arizona, Utah, and formerly California, with more than 16 years serving the Colorado City area. She specializes in trauma recovery and holds a certificate in post-traumatic stress intervention. Her education includes Long Beach State, Chapman College, and United States International University.

Ari Mendez, LCSW
Born in Guatemala, Ari is dedicated to helping individuals and families build healthier lifestyles. He integrates mental and physical health to help patients create lasting positive change. He lives in Utah with his five children.

Eric Derbidge
Eric holds a Master's in Clinical Mental Health Counseling with an emphasis in Marriage and Family Therapy from Grand Canyon University. He's been serving the local community for two years, helping clients recognize inner strengths. Originally from Salt Lake City, he moved to Fredonia in 2000.

Michael DeLeon
Michael is working toward her Master's in Clinical Mental Health Counseling with an emphasis in trauma. She's passionate about empowering clients to reach their goals while honoring cultural awareness and sensitivity.

Trey Esplin, DDS
Dr. Esplin holds a Bachelor's in Nutrition from Southern Utah University and completed dental school at Virginia Commonwealth University. He looks forward to meeting the dental needs of the community.
Physical Therapy

Tyler Farr, PT, DPT, FAS
Dr. Farr received his BS in Athletic Therapy from Weber State University and a Doctorate of Physical Therapy from Ohio University. He has advanced training as a foot and ankle specialist and is an expert in dry needling, manual therapy, and low back pain.

Each board member has a strong love and commitment to our Creek Valley Community. The Board of Directors has representation from Apple Valley, Hildale, Colorado City, Centennial Park, and Cane Beds. Board Members focus their energy on improving the health outcomes in our area, and work hard to ensure long-term sustainability of the organization.
