Benjamin Williams, DO
Dr. Benjamin Williams serves as the Chief Medical Officer at Creek Valley Health Clinic, where he oversees all of our dedicated providers. A native of Colorado City, AZ, Dr. Williams began his journey in family medicine at Mohave Community College and completed his medical education at the Mayo Clinic in Rochester, MN.
With extensive experience in pediatrics, adult medicine, and geriatrics, Dr. Williams previously practiced family medicine in Page, Arizona. He brings a deep commitment to compassionate, community-centered care and is passionate about serving the people of the region he calls home.
Dr. Williams currently lives in Centennial Park with his wife and children. When he’s not in the clinic, he enjoys mountain biking, spending time with his family, and savoring a good cup of coffee. He is a full-time provider at Creek Valley and is currently accepting new patients.

Joanne Yarrish, CNM, FNP
Joanne Yarrish is a Certified Nurse Midwife and Family Nurse Practitioner at Creek Valley Health Clinic, where she oversees much of our OB/GYN care. She provides prenatal and postnatal support, women’s health services, and family planning, and she also works with the Centennial Park Birthing Clinic, offering expectant mothers a trusted, local option for childbirth.
With more than 30 years of clinical experience and over 25 years serving the Short Creek community, Joanne is passionate about empowering women through education, preventive care, and evidence-based practice. She has delivered more than 1,000 babies and is dedicated to supporting women at every stage of life, including helping women over 40 live healthier, fuller lives.
Outside of work, Joanne enjoys reading, traveling, and exploring different cultures, having visited many countries across Europe, Egypt, and Turkey. She is married and has two daughters. Joanne typically sees patients 2–3 days per week and is currently accepting new patients.

Rebekah Williams, PA-C
Rebekah Williams is a Certified Physician Assistant and a graduate of the University of Utah’s Physician Assistant Program in 2023. She brings 14 years of prior experience in Hyperbaric Medicine to her role.
A native of the Short Creek area, Rebekah is deeply grateful to be back home serving the community she loves. She is passionate about providing excellent, patient-centered care and is excited to support the health and well-being of her neighbors.

Marianne Holmes, FNP-C
Marianne Holmes, FNP-C, is a board-certified Family Nurse Practitioner at Creek Valley Health Clinic. She earned her MSN from United States University in 2024 after six years of hospital nursing experience. Passionate about whole-person wellness, Marianne empowers patients to understand their health and make informed choices. A St. George resident, she enjoys backpacking and traveling with her husband and four kids. Marianne is proud to serve her community with compassionate, personalized care.

Matt Lasslo, LPC
Matt Lasslo is a Licensed Professional Counselor and the Director of Behavioral Health. He brings a diverse background in mental health, beginning as a crisis counselor and later working in outpatient behavioral health. He has also served as a clinical director, where he developed programs and guided a team in providing integrated, whole-person care.
Matt has extensive experience working with clients facing depression, anxiety, PTSD, and substance use challenges. He is passionate about integrating mental and physical health to achieve positive outcomes and is deeply committed to providing compassionate, empathetic care.
Originally from Arizona, Matt looks forward to serving the Colorado City community and supporting individuals and families on their path to healing and wellness.

Peggy Taylor, Psy.D.
Dr. Peggy Taylor is a Clinical Psychologist licensed in Arizona, Utah, and formerly in California. With more than 16 years of experience serving the Colorado City area and 20 years in private practice in Temecula, California, she brings a wealth of expertise to her work.
Peggy specializes in trauma recovery and holds a certificate in post-traumatic stress intervention. She has worked in a variety of clinical settings and is deeply committed to helping clients explore their strengths, gain insight, and reach their fullest potential.
A graduate of Long Beach State, Chapman College, and the United States International University in San Diego, Peggy is dedicated to providing compassionate, patient-centered care that fosters healing and resilience.

Ari Mendez, LCSW
Born in Guatemala, Ari Mendez is a licensed behavioral health professional with a passion for supporting individuals and families in building healthier lifestyles and improving overall wellness. He is especially dedicated to integrating mental and physical health, helping patients create lasting, positive change.
Now living in Utah with his five children, Ari enjoys traveling, reading, and hiking in his free time. A fun fact about him: after arriving in the U.S., Ari joined his high school swim team as a senior and was awarded the Most Inspirational Swimmer Award—not for his skills in the water, but for his determination and relentless effort to improve.

Eric Derbidge
Eric Derbidge is a behavioral health provider with a Master’s in Clinical Mental Health Counseling, with an emphasis in Marriage and Family Therapy, from Grand Canyon University. He also holds a Bachelor’s in Psychology from Northern Arizona University.
For the past two years, Eric has been serving the local community as a counselor, helping clients recognize their inner strengths and develop a greater appreciation for life. Originally from Salt Lake City, he moved to Fredonia in 2000 and has spent more than 20 years building relationships, understanding, and love for the culture and communities in the area.
Outside of his work, Eric enjoys spending time with his family, playing board games, watching movies, and exploring new experiences.

Michael DeLeon
Michael DeLeon is a behavioral health provider currently working toward her Master’s in Clinical Mental Health Counseling with an emphasis in trauma. She plans to pursue licensure as a Licensed Professional Counselor (LPC) as she continues to grow in the field.
For the past two years, Michael has been serving the Colorado City community, where she is passionate about empowering clients to reach their goals and live the life they envision. She focuses on highlighting each individual’s values and strengths while honoring cultural awareness and sensitivity.
Outside of her work, Michael enjoys kayaking, creating art, spending time with her family, and hiking near water.

Dr. Trey Esplin, DDS
Dr. Trey Esplin is a Dentist at Creek Valley Dental Clinic. He holds a Bachelor's in Nutrition from Southern Utah University and completed dental school at Virginia Commonwealth University. Dr. Esplin is passionate about outdoor activities like golfing, camping, and riding his dirt bike. Outside of work, he enjoys spending time with his wife and three busy kids. He looks forward to meeting the dental needs of the community.
