At Creek Valley Health Clinic, our mission is to promote healthier lives through affordable, accessible, integrated healthcare.
We have a goal to make our community a better place through providing healthcare services, education, and other vital resources to everyone, regardless of their ability to pay.
We are a 501(c)(3) nonprofit health center managed by a Board of Directors who all live and work in ‘Creek Valley’ (Apple Valley, Hildale, Colorado City, Centennial Park, and Cane Beds). In order to best serve the community, we have recently received a Program 330 grant award to become a Federally Qualified Health Center (FQHC). As a community health center, we help increase access to crucial primary care by reducing barriers such as cost, lack of insurance, distance, and language for our patients.
We are excited to offer annual checkups, well-child services, sports physicals, vaccinations, and other preventive medical services. In addition to primary care, we offer behavioral health services, improved pharmacy access, laboratory services, health education, and insurance eligibility assistance. We also accept same day and walk-in appointments for all of your urgent medical needs.
As a nonprofit community health center:
- Creek Valley Health Clinic will not deny services based on a patient’s inability to pay.
- Creek Valley Health Clinic will not deny services for any reason – including: race, color, sex, national origin, disability, religion, age, sexual orientation, or gender identity.
- Creek Valley Health Clinic offers discounts based on family size and income. We call this our 'HealthShare Program' and is available to all patients, regardless of income or insurance status.
